As the weather cools down, lifting season is officially back.

Today’s before-and-after focuses on a common concern that tends to get more noticeable with age—and often doesn’t improve much with weight loss: double-chin fat.

When we’re younger, skin tends to stay firm (like the left image). Over time, elasticity naturally decreases, and the lower face can start to look looser and heavier (like the right image).

One of the biggest contributors to that “heaviness” is fat under the chin.

Case 1: Woman in her mid-30s

She was still relatively young, and her overall volume and elasticity (including the forehead and mid-face) were in good shape. However, she had localized fullness under the chin, and the lower face felt soft and easily movable—making the jawline look less defined.

Previous treatments

  • Multiple contouring injections: Minimal effect
  • InMode (2 sessions): Minimal effect

When the double chin is this pronounced, it can make you look older even if your nasolabial folds or mouth-area sagging aren’t severe. It can also make the face appear larger and less sculpted than it actually is.

In this case, we used Ultherapy to firm and tighten the jawline, and we added V-OLET (a semi-permanent fat-reducing injection) to help reduce the fullness under the chin.

Before vs. After

To be transparent, Ultherapy and V-OLET aren’t “zero-pain” treatments. You may also experience bruising and swelling, especially when treating the lower face.

Because lower-face contouring is such an important step for improving the jawline, many people consider V-OLET—but if you have frequent social plans, bruising or swelling can make it harder to schedule.

So next, I’ll briefly share a real example of what Ultherapy alone can (and can’t) do for the lower face when we don’t add V-OLET.

Case 2: Woman in her mid-50s

Her top concern was pain. With regular appointments and golf meetups, she didn’t want anything that might cause visible bruising—so V-OLET was off the table. Her goal was to sharpen the jawline using Ultherapy only.

Previous procedure experiences

  • Jawline & salivary gland Botox: No effect
  • Thread lifting: Effective, but too painful to repeat

She visited our clinic looking for a way to achieve a more defined jawline without thread lifting.

She was understandably worried about discomfort, but with local anesthesia, she was able to complete the Ultherapy treatment comfortably.

Before vs. 1 month after

At her one-month follow-up, she told us friends and family had been asking, “Did you do something? Did you lose weight?” She was very happy with the changes—and honestly, hearing that made our day too.

Before vs. After (front view)

Before vs. After (45-degree view)

The jawline, which previously looked a bit soft and undefined, appears firmer and better supported—creating a cleaner, more defined outline.

With the right treatment plan, Ultherapy alone can deliver beautiful tightening results. However, in this case we did not use V-OLET, which specifically targets and reduces double-chin fat.

Before vs. After (90-degree side view)

If we had also broken down the remaining under-chin fullness (the area marked by the arrow in the photo), the result may have looked even more “complete.” That’s simply my personal, honest note as a clinician.

That said, the patient loved how simple and effective Ultherapy felt for her schedule and comfort level—so she decided to maintain her results by coming in regularly (about every six months) and continuing with Ultherapy.

What to expect: discomfort, downtime, and “worth it”

Let’s be real: double-chin lifting and jawline contouring aren’t the kind of treatments you breeze through like a facial. Many patients already know that Ultherapy and contouring procedures can be uncomfortable.

However, compared with surgical liposuction, there’s typically less swelling and you can usually return to daily life quickly.

“No pain, no gain,” right? And one big advantage of V-OLET is that its fat-reducing effect is semi-permanent, which can make the temporary discomfort feel more worthwhile for the right candidate.

Also, pain and swelling can vary widely from person to person. One of our fastest-recovering patients found the treatment so manageable—and the results so noticeable—that she came back just three days later for two additional vials.

One small (and slightly funny) frustration: in our area, V-OLET still isn’t widely recognized. If you search for “Incheon V-OLET,” it often autocorrects to “Incheon Violet,” and you end up with hair salon reviews… which always makes me laugh (and sigh) at the same time.

In our experience, V-OLET results can be truly impressive—yet it’s still far less known than it deserves to be. Maybe the company needs to invest a little more in marketing… (and yes, there I go again!).

Thank you for reading.
