The before-and-after case we are presenting today focuses on an area that tends to accumulate fat more as we age and doesn’t respond to weight loss: the double-chin fat.

When we’re younger, the skin is firm like the left image, but as we age, it gradually sags and appears loose, like the right image.
One of the central areas contributing to this heaviness is double-chin fat.
Case 1: Woman in her mid-30s
She wasn’t significantly aged, and her overall skin elasticity and volume—including the forehead and mid-face—were good. However, she had excess fat specifically in the double-chin area, and the lower face was soft and pliable, giving a sagging appearance.
Previous treatments:
- Multiple contouring injections: Minimal effect
- Inmode (2 sessions): Minimal effect

The area of concern—when the double chin is pronounced like this—can make the face appear older, even if nasolabial folds or sagging around the mouth aren’t severe. It can also make the face look larger than it actually is.


So, while Ultherapy helps tighten the jawline and improve elasticity, we also performed the semi-permanent fat-reducing injection, V-OLET, to reduce fullness.

Of course, Ultherapy and V-OLET contouring injections are not painless procedures.
There can be bruising and swelling.

When performing lower-face procedures for double-chin correction, which are essential for overall contouring, it can be difficult to choose V-OLET if you frequently meet people, due to potential bruising and swelling.
So today, I’ll briefly show the results and possible limitations of performing lower-face correction with Ultherapy alone, without V-OLET.
Case 2: Woman in her mid-50s
Her biggest concern was pain, and she had frequent appointments and golf meetings, so V-OLET wasn’t an option. She wanted to sharpen her jawline using only Ultherapy.
Previous procedure experiences:
- Jawline & salivary gland Botox: No effect
- Thread lifting: Effective but too painful to repeat
She visited our clinic wanting to achieve a sharper jawline without thread lifting.

The patient was very concerned about pain from Ultherapy, but was able to undergo the procedure comfortably with just local anesthesia.

A month later, friends and family asked, “Did you do something to your face? Did you lose weight?” The patient was very satisfied with the results and came back for follow-up, which made me truly happy and grateful.

The jawline, which previously looked slightly loose and undefined, becomes firm and well supported.
In other words, a clear jawline is created.
Ultherapy alone can also produce good results if the treatment is well designed. However, in this particular case, V-OLET—which targets and reduces double-chin fat—was not used.

Looking at the after photo, if the double-chin fat indicated by the arrow ^^! had been broken down just a little more with V-OLET, the overall result might have been even more complete—this is just a small personal note of mine. However, our patient felt that since the procedure was so simple and effective, they decided to continue it regularly every six months and make up for any remaining limitations of V-OLET with Ultherapy.
Double-chin lifting procedures are not exactly treatments you receive in a graceful or comfortable way. By now, many patients already come in knowing that Ultherapy and contouring procedures can be painful. That said, compared to liposuction surgery, there is much less swelling and you can return to daily activities right away.
No pain, no gain!
That being said, one major advantage of V-OLET injections is that the fat-dissolving effect is semi-permanent, so enduring the discomfort is well worth it.
Another interesting point is that pain and swelling vary greatly from person to person. At our clinic, the patient who recovered the fastest actually felt the treatment was effective and manageable—and ended up getting two more vials just three days after the procedure.
What’s a bit disappointing is that in our area, there still don’t seem to be many clinics that appreciate V-OLET as much as we do. When you search for “Incheon V-OLET,” it gets automatically corrected to “Incheon Violet,” and all that comes up are hair salon reviews… which honestly makes me quite sad.
The effects of V-OLET injections are really quite good, but they seem to be far less well-known than they should be. I think the V-OLET company might need to put in a bit more effort with their marketing… (there I go criticizing again).
